When you need to restore the mobility you lost, Functional Transformation Mobile Rehab is here with our standard and specialized services. We can help you get back to performing daily tasks as independently as possible with little to no pain.
We’re a Medicare Part B-approved provider. You don’t need to be home-bound to use our services. If you are a traditional Medicare Part B patient, expect to pay no more than you typically would at a brick-and-mortar facility. If you have secondary insurance, there are no out-of-pocket costs.
We also work with a few select insurance providers. As of August 2021, we are a United Health Care in-network provider and are waiting for in-network accreditation from Humana, Cigna, and Aetna. Until we receive the accreditations we are considered out-of-network.
