Functional Transformation Mobile Rehab is a Medicare Part B-approved provider. We are not considered to be a home health service, and patients don’t need to be home-bound in order to use our services. If you are a traditional Medicare Part B patient, you can expect to pay no more than you typically would at a brick and mortar facility. If you are a patient with secondary insurance, you can enjoy no out-of-pocket costs.
As of August 1, 2021, Functional Transformation Mobile Rehab is considered to be an in-network provider for UHC. For all other insurances, we are out-of-network. However, we are currently working on credentialing with HUMANA, CIGNA, and AETNA. If you are worried about cost or coverage, our team can gladly assist you before we start any official work.

No, absolutely not. This is the most common point of confusion. Traditional "Home Health" (Medicare Part A) requires you to be strictly homebound to receive care.
Because Functional Transformation Mobile Rehab operates as a Mobile Outpatient Provider (Medicare Part B), we bring the traditional outpatient clinic experience directly to your front door. You do not need to be homebound. You can still drive, go to the grocery store, and live your normal life—you simply choose to receive your expert care in the comfort and privacy of your home rather than traveling to a clinic.
