Doing these exercises is beneficial for staying active in daily life.

Functional Transformation Mobile Rehab is a Medicare Part B-approved provider. We are not considered to be a home health service, and patients don’t need to be home-bound in order to use our services. If you are a traditional Medicare Part B patient, you can expect to pay no more than you typically would at a brick and mortar facility. If you are a patient with secondary insurance, you can enjoy no out-of-pocket costs. 

As of August 1, 2021, Functional Transformation Mobile Rehab is considered to be an in-network provider for UHC. For all other insurances, we are out-of-network. However, we are currently working on credentialing with HUMANA, CIGNA, and AETNA. If you are worried about cost or coverage, our team can gladly assist you before we start any official work.

A home aide or companion is hired to do tasks for you (such as cooking, cleaning, or dressing you).
As Licensed Occupational Therapists, our goal is the exact opposite: we work one-on-one with you to modify movements, build strength, and adapt your environment so you can perform those daily tasks safely and independently on your own. We look at everything from your ability to safely transfer onto a toilet to managing your kitchen tasks without falling.

Yes, absolutely. When limbs are chronically swollen, the skin becomes incredibly fragile, stretched, and starved of healthy blood flow. This makes you highly susceptible to open sores, weeping skin (lymphorrhea), and dangerous skin infections like Cellulitis.
Because we combine specialized wound care with our mobile therapy, we don't just patch up a sore. We treat the underlying fluid accumulation that is causing the skin to break open in the first place. We apply sterile, advanced dressings that comfortably fit under compression, monitor closely for early warning signs of infection, and teach you how to protect your skin right at home to prevent wounds from coming back.

No, absolutely not. This is the most common point of confusion. Traditional "Home Health" (Medicare Part A) requires you to be strictly homebound to receive care.
Because Functional Transformation Mobile Rehab operates as a Mobile Outpatient Provider (Medicare Part B), we bring the traditional outpatient clinic experience directly to your front door. You do not need to be homebound. You can still drive, go to the grocery store, and live your normal life—you simply choose to receive your expert care in the comfort and privacy of your home rather than traveling to a clinic.
